This document lists the due dates for submitting all documentation for payroll processing

       

Required New Hire Forms


If the Diocesan Office is responsible for processing payroll for your congregation, all of the following forms, unless otherwise stated, are required to establish their employment record.

Please Note: Use this form if you would like a different amount withheld for the state of Oklahoma taxes than for federal taxes; or if you would like an additional amount withheld for the state of Oklahoma taxes.)

Status Change Form (Excel file)
Use this form to notify the Diocesan Office of any changes in personal or employee information to ensure that all records (including benefits) are updated.

     

Employee Medical and Dental Benefits

All medical and dental benefits for eligible employees in our diocese are provided through the Church Pension Group. 

If you have any changes to employee benefits, please contact Martha Smith, Payroll & Benefits Coordinator, directly via phone at (405) 232-4820 or via email at MSmith@epiok.org

Church Pension Group: Once enrolled, you will be able to access your benefits information and manage your account through your MyCPG account on www.CPG.org. You can also contact CPG directly by phone for questions regarding medical, dental, and vision coverage at (800) 480-9967 or through the main customer service number at (800) 223-6602.

Employee Pension Benefits

All pension benefits for eligible employees in our diocese are provided through the Church Pension Group and administered through Fidelity Investments. 

If you have any changes to employee pensions, please contact Martha Smith, Payroll & Benefits Coordinator, directly via phone at (405) 232-4820 or via email at MSmith@epiok.org

Fidelity Investments: Once enrolled, you will be able to access your pension information and manage your account (including updating personal information, applying for personal pension loans, tracking your pension contributions, and more) through your account on www.fidelity.com or by calling them directly at (877) 208-0092.

Church Pension Group: Additionally, once enrolled, you can view a summary of your pension benefits information through your MyCPG account on www.CPG.org. You can also contact CPG directly by phone for questions regarding pensions, life Insurance, annuities, retirement savings, and disability at (866) 802-6333 or through the main customer service number at (800) 223-6602.

 

Retirement
